Other Worksystems Policies

Worksystems has some policies that apply systemwide to all of its Regional Program Standards and Contract Statement of Works.

SSN Disclosure & EEO Grievance Forms

The EEO Grievance Form serves two purposes:

  1. Public Posting: A version of the EEO Grievance Notice, including contact information for Worksystems’ Compliance Manager, Kari Brenk, must be posted in a publicly accessible location at each provider organization.
  2. Applicant Review: A version of the EEO Grievance Notice, including the provider’s EEO officer contact information, must be verbally reviewed with each program applicant.

The EEO Disclosure Form, with the completed provider manager’s contact information, can be printed double-sided along with the Standard SSN Disclosure Form to distribute and review with applicants during the enrollment process. If the enrollment is conducted virtually, these disclosures must be emailed to the applicant.

Background Check Requirements

Worksystems Contract Statement of Work and Regional Program Standards contains criminal background check requirements for direct service staff, volunteers and subcontractor staff that work with participants enrolled in a Worksystems-funded program.

Participant Release

Worksystems requires participants to consent to us sharing this story or image. Participants must sign a release of information for photos, videos and success stories, and contractors must keep the signed participant release in the participant’s file.

Record Retention

Worksystems requires contractors to maintain participant records for set periods of time.
