Equal Opportunity (EO) Is the Law and Social Security Number (SSN) Disclosure Forms
Worksystems Regional Program Standards contain requirements to provide a copy of, and verbally review, the EO and SSN Disclosure Forms with all applicants. The EO and SSN Disclosure Forms can be printed double-sided to distribute and review with applicants during the enrollment process. If enrollment is conducted virtually, these disclosures must be emailed to the applicant.
An ‘Equal Opportunity Is the Law’ Posted Notice must be posted in a publicly accessible location at each subrecipient provider location where applicants and participants access services.
Reasonable efforts must be made to ensure that the two Disclosure Forms and the Posted Notice are understood by all applicants, including youth, and those who are limited-English speaking individuals. These efforts must comply with the language requirements of 29 CFR 37.35 regarding the provision of services and information in languages other than English.

Background Check Requirements
Worksystems Contract Statement of Work and Regional Program Standards contains criminal background check requirements for direct service staff, volunteers and subcontractor staff that work with participants enrolled in a Worksystems-funded program.

Participant Release
Worksystems requires participants to consent to us sharing this story or image. Participants must sign a release of information for photos, videos and success stories, and contractors must keep the signed participant release in the participant’s file.

Record Retention
